A person, known as the Affiant, should fill out the document. He must be able to take an oath that he personally knows the circumstances of the subject's birth. Usually, this person is a relative, a parent or a grandparent, a doctor, or a close family friend. This document must also contain information identifying the Affiant, including his name, address, and birth date.
This document also contains information identifying the subject of the Affidavit - name, gender, age, and citizenship. The paper concludes by explaining how the Affiant and the subject know each other and adds evidence of the Affiant's personal knowledge of the circumstances surrounding the subject's birth.
Once completed, the document should be signed and notarized. The subject of the Affidavit should keep the notarized document in a safe place, in case he needs to prove his birth in the future.
No law regulates what should be described in the Affidavit of the Birth. However, it is acceptable for the information provided to be specific and convincing to the authorities about the subject's birth.
